You’ve probably seen it everywhere lately — influencers, longevity doctors, and wellness brands calling hormone therapy + GLP-1 meds (like semaglutide or tirzepatide) the “ultimate midlife stack.”
The promise? Effortless weight loss, balanced hormones, youthful energy, and a total body reset.

But let’s slow down and take a look at what’s actually going on under the hood.

The Science (What We Know So Far)

Hormone Therapy (HT)
Hormone therapy — whether it’s estrogen, progesterone, testosterone or a combination — is well-studied and can offer significant benefits for women in perimenopause and beyond: improved sleep, bone health, mood, and even cognition. When used appropriately and monitored, it’s one of the most effective ways to manage menopausal symptoms.

GLP-1 Medications
GLP-1 receptor agonists (like Ozempic®, Wegovy®, Mounjaro®) were designed to manage blood sugar and improve insulin sensitivity in people with diabetes. Their ability to reduce appetite and promote fat loss made them headline-grabbing weight-loss tools — and they’re now being used widely, including among women navigating midlife weight changes.

The Stack
Combining HT and GLP-1 isn’t necessarily a bad idea — both address overlapping issues of metabolism, body composition, and inflammation. But here’s the reality:

  • There are no long-term studies on how these medications interact in peri- or postmenopausal women.
  • We don’t yet know the full metabolic ripple effects, especially as hormones fluctuate.
  • Some early clinical experience suggests women may need different hormone dosages once GLP-1s alter how the body metabolizes fat and estrogen.

Now let’s talk marketing.

Wellness culture loves a shiny new stack.
“Optimize your hormones.”
“Reboot your metabolism.”
“Look 30 again.”

But the marketing skips a few details:

  • GLP-1s can lead to muscle loss, not just fat loss - especially if we aren't focusing on resistance training.
  • They slow digestion paired with nausea, which can affect how your hormone therapy is absorbed. Our gut is a hormone recycling center - and this unfortunately is a common side-effect.
  • Appetite suppression often leads to less protein and mineral intake — the very nutrients required to support your foundational health.

It’s not that these tools don’t work — they do. But they work best when you’ve already built a solid foundation: blood sugar stability, consistent protein/fiber/carbs, minerals, resistance training, and rest.

HT and GLP-1s operate in the same eco system and it requires a special lens to ensure you are supported.

The real risks (and the stuff no one is talking about):

💥 Rebound weight gain when GLP-1s stop — unless you’ve rebuilt your metabolic powerhouse (this is very nuanced and not as simple as protein and weights).
💥 Mood dips and fatigue as nutrient intake drops.
💥 Hormone chaos as body fat (and therefore estrogen storage) shifts.
💥 Digestive slowdown that changes how your meds work and how your body metabolizes estrogen.
💥 Long-term cost and dependency — because “maintenance mode” often means staying on it indefinitely which is absolutely fine as long as you are making an informed decision.

Here’s the truth:

HT and GLP-1s aren’t villains — they can be incredible tools when used strategically. (Many of our clients have been successful on both HT and GLP-1 medications.)

But when you add in influencers flaunting their “menopause transformation” without disclosing the support systems (nutrition, strength training, foundational health), and it’s easy for women to think this is the answer.

Our metabolism doesn’t need to be hacked.
It needs to be supported — with food that nourishes, minerals that restore you on a cellular level, muscle that protects, and a nervous system that’s no longer running on fumes.

We remind our clients:

  1. Get curious - it is Ok to explore different types of treatments
  2. Ask yourself - is this right for my body at this stage
  3. There will always be trends
  4. Get to you know yourself - instead of Googling or asking AI for recommendations - take data on yourself.
